Position Summary We have an immediate opening for a compassionate and skilled physician to join our Palliative Care Program, with a primary focus on building and expanding a robust community?based model of care. This role includes providing consultation services to patients and families, supporting symptom management, and helping guide complex medical decision?making. The position also includes participation in a shared rotation providing palliative care consults at a partnering local community hospital.
The Palliative Care Physician serves as a key medical resource to the interdisciplinary team, ensuring the highest standards of quality, communication, and continuity of care. This individual will support system-wide palliative care initiatives, champion best practices, and contribute to the development of innovative program strategies that improve patient outcomes and enhance quality of life.
The physician will supervise the nurse practitioner assigned to the program and is responsible for oversight of clinical practice, professional development, and collaboration between disciplines. In addition, the physician provides direct consultations, follow?up visits, and ongoing management for patients receiving community-based palliative care services.

Responsibilities
Provide comprehensive palliative care consultations to patients in homes, facilities, and occasionally in hospital settings.
Conduct follow?up visits focused on symptom management, pain control, goals of care, and advance care planning.
Partner with the interdisciplinary team—including nursing, social work, spiritual care, and clinical leadership—to develop individualized care plans.
Serve as the primary medical leader for the community?based palliative care program, guiding clinical direction and workflow.
Supervise, mentor, and provide clinical oversight to the program’s nurse practitioner(s).
Collaborate with referring physicians, specialists, and hospital teams to support smooth transitions of care.
Participate in rotating on?call responsibilities as needed to ensure continuity of care.
Maintain accurate and timely documentation in alignment with Medicare, CHAP, and organizational standards.
Provide education to staff, community partners, and the public on palliative care principles and best practices.
Support quality improvement initiatives, data review, and evaluation of program outcomes.
Promote patient and family-centered care, honoring individual preferences, cultural values, and goals.
Requirements / Qualifications
Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) with an active Pennsylvania medical license.
Expertise in the medical management, symptom control, and pain management of individuals with serious or terminal illness.
Board certification or eligibility in Internal Medicine or Family Medicine preferred.
Accreditation or certification in Hospice and Palliative Medicine (AAHPM or equivalent) strongly preferred.
Demonstrated experience in interdisciplinary teamwork and communication with patients, families, and care partners.
Strong clinical judgment, empathy, and commitment to high-quality patient-centered care.
Ability to work independently in community settings and manage a mobile clinical schedule.
Valid driver’s license and reliable transportation for travel between care sites.
