Job Summary and ResponsibilitiesDignity Health Medical Group - Arizona is seeking a full-time Geriatrician to join our busy department in Prescott, Arizona
- Patient population includes Geriatrics, Dementia, and Palliative Care
- Collaborate closely with interdisciplinary teams and Advance Practice Providers to enhance efficiency and care coordination to ensure comprehensive patient care
- Actively engage in patient education and shared decision-making to empower older adults and improve health outcomes
- Serve as a Geriatrics Consultant, including screening and diagnosing Dementia
- Clinic position with possibility of home visits
- Full time position, Monday through Friday
- Provider will have 4 hours of administrative time per week
- Daily patient volume: anticipated 6-10 patients per day
- Current programming includes multi-disciplinary Dementia Screening Clinic, Alzheimer's & Dementia Care Program (UCLA dissemination site), Outpatient and Inpatient Palliative Care, and GUIDE model participant

Job Requirements- BE/BC in Geriatric Medicine (IM or FP), Neurology, or Hospice & Palliative Medicine (IM or FP)
- Experience or interest in working with Dementia/Alzheimer's Disease population
- Strong communication and interpersonal skills
- Valid state medical license as MD or DO
- Valid DEA certificate
- Strong clinical skills and experience working with older adults
- Ability to work effectively in a team-based healthcare environment
- Demonstrate empathy, compassion, and respect for the unique needs of older adults and their care partners
