Learn more at www.multicarejobs.orgPosition Summary:The After Hours Nurse is a Registered Nurse responsible for providing healthcare triage assessment, direct nursing care as needed/assigned in a home care setting, and providing the initial assessment, evaluation, and care planning for the patient after normal scheduled office hours. Nursing care is based on an individual plan of care as ordered by the physician and monitored by the Hospice Interdisciplinary Team (IDT).Minimum Requirements:
