Carle Health is seeking a Nurse Practitioner to join our growing Hospice and Home Health team in Peoria, Illinois.Position Details Include:PRN positionThe Hospice NP will collaborate with the interdisciplinary Hospice/Palliative Care team in the assessment of patients and comprehensive plan of care, and management of patients who have advanced progressive chronic illnesses.
