At Centura Health, we value incredible people with incredible skills - but your commitment to a greater cause is something we value even more.   Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients; linkage to resources in the community.What You Bring/Job Requirements:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:   Master of Social Work (MSW).6 months home care/hospice experience preferred.Two years post Master's experience in health care or related field preferred.Licensed Clinical Social Worker (LCSW) preferred.Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from American Heart Association required for facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED. Current Driver's licenseMedium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly This is for a PRN day shift (8-5pm).  We do rotate coverage on holidays.   We are patient and family focused with a highly collaborative team.  We have heavy volumes with the flu season.  Typically we will see mostly adult patiens with some NICU patients.   Your Connected Community:When people and places align, incredible things happen.
