Loyola Medicine is seeking primary care physicians boarded in Family Medicine or Internal Medicine to be part of our House Calls program, making home visits in the suburbs of Chicago and western Cook County, Illinois.

Full-time and part-time positions are available.

The measure of success for this position is improved patient healthcare outcomes, leading to a lower admission and readmission for this patient population. This position will part of a team, with nursing and social work support, managing a panel of homebound patients.
