The Access Psychotherapist/Care Coordinator provides timely intervention, brief treatment, case/care coordination, and care transition support for Veterans as a member of the Access Team within the Behavioral Health Service Line (BHSL) of the VA Southern Oregon Rehabilitation Center and Clinics (VA SORCC). This position is based at the main campus in White City Oregon, with occasional travel to the community based outpatient clinics in Grants Pass & Klamath Falls, Oregon. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Candidates must meet one of the following: Hold a master's degree or doctoral degree in marriage and family therapy from a program approved by COAMFTE, Hold a master's degree or doctoral degree in marriage and family therapy from a MFT program from a regionally accredited institution, OR, Education. Hold a master's or doctoral degree in: Clinical Mental Health Counseling; Clinical Rehabilitation Counseling; Clinical Mental Health Counseling and Clinical Rehabilitation Counseling; or a related field, from a program accredited by the Council on Accreditation of Counseling and Related Educational Programs (CACREP). Examples of related mental health counseling fields include, but are not limited to Addiction Counseling; Community Counseling; Gerontology Counseling; Marital, Couple, and Family Counseling. CACREP defines the date when graduates are considered to have graduated from a CACREP accredited program. Additional information may be obtained from http://www.cacrep.org/directory/. NOTE: Traditional Rehabilitation counseling programs that are accredited by CACREP do not meet the LPMHC qualification standards as Traditional Rehabilitation counseling differs from Clinical Rehabilitation counseling. OR, Hold a master's degree or doctoral degree in a comparable mental health degree (Licensed Professional Mental Health Counselor, Social Work, Psychiatric Nursing, Psychology, and Psychiatry) that meets the current VA qualification standard of that profession. Licensure. Licensure. Persons hired or reassigned to LPMHC positions in the GS-0101 series in VHA must hold a full, current, and unrestricted license to independently practice as a Licensed Professional Mental Health Counselor, which includes diagnosis and treatment. OR Persons hired or reassigned to MFT positions in the GS-0101 series in VHA must hold a full, current, and unrestricted MFT license to independently practice marriage and family therapy in a State. Exception: The Secretary, or his/her designee, may waive the licensure requirement for persons who are otherwise qualified, pending completion of State prerequisites for licensure examinations for a period not to exceed three years from the date of employment on the condition that MFTs appointed on this basis provide care only under the supervision of a fully licensed MFT with an AAMFT approved license. Non-licensed MFTs who otherwise meet the eligibility requirements may be given an appointment as an MFT under the authority of 38 U.S.C. § 7401(3). This exception only applies at the entry grade level (GS-9). For grades at or above the full performance level, the candidate must be licensed. Grade Determinations: Senior Licensed Professional Mental Health Counselor, GS-12: (a) Experience, Education, and Licensure. In addition to the basic requirements, candidates must have at least one year of progressively complex LPMHC experience equivalent to the GS-11 grade level. Senior LPMHCs must have five years of post-licensed experience that demonstrates possession of advanced practice skills and judgment, demonstrating progressive professional competency and expertise and be licensed to provide clinical supervision to trainees or unlicensed LPMHCs. Demonstrated Knowledge, Skills, and Abilities. In addition to meeting the KSAs for the GS-11 grade level, the candidate must demonstrate all of the KSAs below: 1. Advanced knowledge of and mastery of theories and modalities used in the specialized treatment of complex mental illness. Ability to incorporate complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Ability to determine priority of services needed and provide specialized treatment. 2. Advanced and expert skill in a range of specialized interventions and treatment modalities for Veterans with complex needs. This includes individual, group, and/or family counseling, or psychotherapy and advanced level psychosocial, and/or case management interventions used in the treatment of Veterans. 3. Ability to coordinate the delivery of specialized psychosocial services and programs. Ability to design system changes based on data. 4. Ability to provide subject matter consultation to colleagues and trainees on the psychosocial treatment of patients, rendering professional opinions based on experience, expertise and role modeling effective clinical skills. 5. Ability to teach, mentor staff and trainees, and provide supervision for licensure or for specialty certifications. 6. Ability to engage in written and oral communication with leadership/staff and community stakeholders regarding policies, procedures, practice guidelines, and issues pertaining to the practice of the profession. ["Psychosocial/Psychotherapeutic Treatment and Intervention: Provides assessment and treatment of Veterans' mental health and substance abuse disorders, with the goals of quickly initiating care for Veterans who are new to the system or in transition between services or providers; assessing Veterans' needs; planning and providing care; and maximizing health outcomes and decreasing morbidity due to substance-related and mental health conditions. Incumbent's role has a direct and significant impact on the effectiveness of health care delivery to the patient population. Incumbent continuously improves individual and organizational provision of mental health care, with focus on Veterans in transition, in accordance with the agency's and Veterans' goals. Specific services include, but are not limited to: - Provides initial functional assessments of Veterans' mental health and substance abuse needs. - Screens Veterans for risk of suicide or harm to others, and intervenes when indicated using evidence-based safety enhancement strategies; -Provides comprehensive, multi-disciplinary treatment planning, with focus on integrative treatment for Veterans with co-occurring addictions and mental health disorders; - Provides time-limited individual and group psychotherapy for a full range of Veteran patients with diverse clinical needs; - Provides Evidence-Based Psychotherapies appropriate to patients' needs; - Provides evaluation and treatment through tele-health technologies, as clinically and administratively indicated; - Assists Veterans in crisis, in collaboration with the Suicide Prevention Team and others. Facilitates referrals for patients in crisis, when indicated. - Monitors assigned Veterans closely, working with team to see that changes in status receive the indicated clinical attention. Documents care provided in CERNER, in full compliance with agency guidelines. Care Collaboration, Coordination, and Consultation: Actively coordinates evaluation and treatment activities with other members of the Access team in support of Veterans' transitions into care. Actively coordinates care with mental health staff and programs in Primary Care, Behavioral Health Interdisciplinary Program (BHIP), the Homeless Outreach, and other VA and non-VA settings to facilitate referrals and transitions to appropriate services. Works actively with VA and non-VA mental health treatment teams, Veteran family members, and others invited by Veterans to participate in their care. - Develops, implements, monitors, and revises initial plans of treatment that are based best practices for Veterans assessed needs and preferences.- Facilitates warm-handoff transitions to VA and non-VA health care services as well as psychosocial support agencies - Actively collaborates with other SATP and BHIP team members to assure seamless transitions in care, with attention to Veteran safety and continuity of care; - Actively coordinates and communicates with Veterans' primary care teams and medical specialists (e.g. nutrition, palliative care, hospice) regarding Veterans' care plans and needs; - Collaborates outpatient services to support discharge planning and care transitions for Veterans new to mental health or substance abuse services. - Collaborates with military units, community agencies, and other non-VA partners to assure seamless transitions into care for Veterans not yet receiving VA care. Works closely with Healthcare Administration eligibility staff and Primary Care to facilitate timely entry into care - Establishes and maintain networks, both within VA SORCC, local CBOC's, other VISN20 sites, and agencies within the White City catchment area. Actively collaborates with these agencies to support expeditious and seamless care transitions for Veterans new to the VA or to MH services.
