ChristianaCare is looking for qualified NP/PA to join our newest locations in Southern Delaware. The new sites will be located in Rehoboth and Milford, opening Summer 2022. In collaboration with Highmark Medicare Advantage, this unique model of care focuses on individualized care plans for patients 65 years old and older. An opportunity to make patient's lives more manageable with standardized clinical outcomes in a Medicare Advantage product. This practice offers unique services designed specifically for the geriatric populations such as a Functional Assessment Room, therapy for ADL support, on-site access to a pharmacist, social worker, nutritionist, as well as a private family gathering room. Provide care focused on quality of life and maintaining function. Innovative, team based primary care provided in office, virtual and home-based settings to a growing community. By leveraging technology and the support of our on-site digital ambassador, we will provide a large virtual component. In addition, this practice will evaluate and treat eligible patients in the comfort of their own home.

CORE DUTIES AND RESPONSIBILITIES

  • Perform physical exams; formulate a diagnosis and develop & execute an appropriate treatment plan; and ensure adequate follow-up care.
  • Independently analyze patient data and create an individualized care plan based on patient assessment and needs.
  • Collaborate with physicians, nursing, and other members of the disciplinary team.
  • Evaluate and monitor the effectiveness of care plan.
  • Appropriate referrals to social work, behavioral health, physical and occupational therapy for
  • Identifies end of life patients, has a basic knowledge and skill set for palliative care and appropriately refers patients to hospice care
  • Utilizing evidence-based medicine coupled with expert clinical knowledge to educate and promote well-being, preventative care, and chronic care management.
  • Helps coordinate transitions of care to maximize quality of life and functional status for the patient and family.
  • Contribute to program development and utilization of best practice guidelines and process improvement.

EDUCATION AND EXPERIENCE REQUIREMENTS

  • Completion of a master's degree program and board certification as a Family Nurse Practitioner, or Adult Nurse Practitioner or completion of a Physician Assistant program and board certification as a Physician Assistant.
  • Two to three years of experience with adults, preferably in memory loss and/or with seniors.
  • BLS Certification
  • Travel between practices and patient homes. Facilities, as needed.
