RN CLINICAL LIAISON/HOME HEALTH CARE
Position Summary:Sign-on Bonus available to experienced external candidates! This role will work closely with the Director and the Clinical Managers. The intent is that the HHC agency (hospital based) has a connection to the care management team for UNMH. This role will assist with difficult home health placement of complex patients and serve to educate about UNMH Home Care services.In home observations will also be a part of this role to support the field staff in practice.
